Dependent Eligibility Verification - Required Birth Certificate Elements
Not every birth certificate is the same - different states have different formats. When utilizing birth certificates to demonstrate proof of eligibility for a dependent verification audit, we require the birth certificate to show you, your spouse, or your domestic partner as the child's parent.
In instances where those required elements are not available, we may also accept:
- report of birth abroad of a citizen of the United States of America (issued by the US Department of State) showing you, your spouse, or your domestic partner as the child's parent; or,
- adoption placement agreement, petition for adoption or adoption papers approved by the court (with signature or seal visible on the photocopy) that shows you, your spouse, or your domestic partner, as the adoptive parent; or,
- divorce decree that lists children adopted during the marriage. Must have court signature, stamp, or seal visible on the photocopy; or,
- affidavit of parentage or paternity (or like form) that acknowledges your, your spouse's or your domestic partner's paternity of the child. The form must have some indication that it was filed with the state or with the court.
