I just got married and added my spouse to my benefits. What documents do I need to provide to verify eligibility?
Congratulations! As a newlywed you would not have the required tax return showing "married filing jointly" or "married filing separately." When you do not have the tax return to prove relationship, we ask for documentation from one of each of these two categories:
1. Proof of Marriage
- marriage certificate; or,
- printout of online marriage record (available in some states and countries) showing your name, your spouse's name, and the date of marriage).
AND
2. Proof of Eligibility
- Affidavit of Eligible Spouse
